Step 1 - Docketwise Intake Form


You will receive an invitation from docketwise to fill out the candidate intake sheet - it is VERY important to first fill this out as much as you can.

after making attempt to fill out forms - please email adjustment@oaklg.com with remaining questions. if you do not have any questions, you will email you if the team needs any more information on the forms.


If you did not get an email for docketwise.com within 24 hours please email info@oaklg.com with details. Please check spam folder first, if you do not see the email in 24 hours during the weekday. If it is a weekend, please wait until close of business on Monday.


Step 2 - Please fill out Document Checklist Portal

Step 3 - Schedule Visit with Medical Doctor and Bring Form I-693


After we have the form intake filled out, we will send form I-693 for you to take with you to a USCIS approved doctor

You will need to Bring the following items to the appointment


  • Form I-693, Report of Medical Examination and Vaccination Record
  • Government-issued photo identification, such as a valid passport or driver’s license. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.
  • Vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease)
  • Medical insurance card (check with the doctor’s office to make sure the office accepts your medical plan)
  • Payment (check with the doctor’s office about acceptable forms of payment). It is also a good idea to call a few doctors to find out how much they charge for the exam. Prices can vary by a few hundred dollars.


During The Exam


The medical examination entails a review of your medical history and a physical examination. The doctor will complete a comprehensive examination that will include a review of your medical history and a physical examination. The doctor will also test for communicable diseases such as tuberculosis, syphilis, and gonorrhea.


After the exam, the doctor will complete Form I-693 and seal the form in an envelope for you to submit to USCIS. Make sure you get a copy of the completed Form I-693 for your personal records before the doctor seals the envelope. USCIS will not accept the form if the envelope has been opened or altered.

After The Exam - Send File and Passport Photos to our Charlotte, NC Office


After the exam, you will need to physically male the I-693 from the doctor to our office along with passport photos of yourself and family members.

Please submit 6 passport style photos of each applicant in the same mailing as form I-693 to the address below:
